A good report is part of the product.

Include	Example
Version	Release tag, package version, or commit
Environment	OS, runtime, deployment shape, relevant hardware
Workload	Document shape, indexes, batch size, concurrency
Expected / actual	What should happen and what happened instead
Evidence	Logs, minimal reproduction, or a link to a public artifact
